Divisions: 10-12yrs, 13-15yrs, 16-Sr. High, 16+ Women, 19-30yrs, 31+

Games begin on Monday, July 25 - Finals are held Thursday, July 28
Slam Dunk Contest - Thursday, July 28

Complementary tickets given for 2nd round matches
(advance tickets, sold until July 21, are $6)
Roster may consist of four people

Release Forms
Minors must have release forms signed by parent
All players over 18 must sign a release form
Release Forms are available on the entry form

Entry Fee
Entry Fee is $36 per team & must accompany entry form
(Checks payable to: Elkhart County 4-H Fair)
Entries limited to the first 110 teams
